New Patient Intake
New patients are asked to arrive 20 minutes prior to their appointment time to complete necessary registration paperwork.

Existing Patients
Medical Records
Access Medical Clinic requires a signed Medical Records Release Form in order to obtain a copy of your medical records or to have your medical records sent to another health care provider. Patients may be charged a fee per page for a printed copy of their medical records.
Access Medical Clinic will not charge a fee if the patient requires this information for a claim for benefits under the Social Security Act or any other state or federal needs-based benefit program.
Prescription Refills
Every effort should be made to address medication refills during your regularly scheduled office visit. If it becomes necessary to refill your medication between appointments, medication refills will only be available during regular office hours and cannot be made after hours, on the weekends or on holidays.
A 24-hour notice is required for all medication refills and patients are asked to plan accordingly. Chronic or long-term medication refills may require an office visit before they can be refilled.
