What happens during an annual Medicare wellness exam?
Your visit will begin with a review of your medical history and an assessment of your health risks, including your health status, risk of injury or depression, lifestyle behaviors, and any urgent care needs you may have. We will create a list of all your current healthcare providers, medical devices, prescriptions, and over-the-counter medications and supplements to ensure we meet your healthcare needs.
You and your Access Medical Clinic provider will work together to create a screening checklist to include tests and evaluations based on your individual preventive healthcare needs. Your height, weight, and body mass index will be measured and recorded, and you may have a simple screening test to look for signs of cognitive impairment. In addition, we will assess your functional ability to perform regular tasks without falling or incurring other injuries. Finally, you’ll be provided with guidance about how to maintain or improve your health, as well as referrals to other healthcare providers as needed.

Access Medical Clinic accepts Medicare, Medicaid, most private commercial insurance plans, cash, check, and debit/credit cards. We ask that patients pay for services at the time they are rendered. Please contact Access Medical Clinic or your insurance provider before your first visit to check if you are covered.
No one will be denied access to services due to an inability to pay; there is a discounted/sliding fee schedule available based on family size and income.
